Need Help with utility bills?
Please bring photocopies of the following documents:
Proof of household’s total gross income, Birth Certificate, ID(drivers license or state ID), Supplemental medical insurance, Social security card, electric bill, gas bill, water and sewer bills (if applicable), lease agreement (if renter) and bring copies of all listed items for ALL members of your household. A photocopier will be on site to make copies, if needed.
